Financial Policy

It is also your responsibility to have a valid health insurance identification card at the time of your visit. We have the ability to verify your healthcare insurance coverage on-line within a matter of minutes. If you do not produce an insurance card or if your coverage is not active at the time of your visit, you will have the option to either reschedule your appointment, or to pay a “good faith estimate” of charges for all expected services before you will be seen by the physicians/providers.
 
All applicable co-pays/co-insurances, fees and deductibles are due and will be collected at the time of service.
You are responsible for any charges not covered or reimbursed by your insurance policy.
 
For your convenience we accept cash, checks and most major credit cards and debit cards.
 
Altamonte Medical Group will assess finance charges to any unpaid balances that exceed 30 days from the date that services were provided.
 
Any fee listed above that is charged to your account must be satisfied before your next scheduled appointment with
a healthcare provider. We reserve the right to refuse non-emergent care to any of our patients who have outstanding balances.
 
We do offer a discount program for uninsured patients, and will be happy to discuss any special considerations in the handling of your account.

 

Billing Office

We are preferred providers for most major health insurance plans. For your convenience, we will be happy to electronically file your primary and secondary insurance claims directly from this office.
 
We encourage you to be familiar with the policy benefits as outlined in your healthcare insurance contract. Should you have any questions regarding your plan’s benefits, or the payment of your claim as detailed on the Explanation of Benefits (EOB) that you receive from your insurer, you are advised to directly contact the Customer Service division of your healthcare insurance company.
